Use the forms below to create a new 4th year elective or provide access for a new elective chair or coordinator.
Complete the 4th Year Elective Proposal by providing your course profile information in the PDF document below. The course chair must have UCLA faculty status and is responsible for course content as well as submitting the written student evaluation. All 4th year electives are approved by the 4th Year College Chair Committee, Medical Education Committee and the UCLA Department Chair before inclusion in the 4th Year Elective Catalog.
A Statement of Understanding is required for all faculty and staff that will be involved with maintaining student rosters and/or managing/submitting student evaluations.
