Vision Service Plan - Prepaid Vision Care

How VSP Works
VSP has made arrangements with numerous optometrists and ophthalmologists
(who are called "VSP Panel Doctors") to provide certain vision care services to
you and your covered dependents. Panel doctors bill VSP directly, so you have limited
out-of-pocket costs for covered services and materials (except for the $10 deductible
and an additional $25 if the doctor prescribes lenses or frames, or both; contact
lenses will cost more). Any additional services and materials not covered by VSP may
be arranged between you and your doctor.
To ensure the quality of service you receive, VSP maintains high professional
standards for its Panel Doctors. Upon request, VSP will send you a list of Panel
Doctors in your area.
If you or your dependent decides to use a doctor who is not associated with VSP, the
plan will pay only a limited portion of the expense.
