Vision Service Plan Plan Coverage Extra Cost
Non VSP Doctor Non Coverage How to Use

What You Pay

You pay the first $10 in covered expenses per person for each eye examination, and an additional $25 if the doctor prescribes lenses, or frames, or both, in any 12 month period and if you use a Vision Service Plan provider. Oversized lenses, designer frames, tints, etc., can be obtained at an additional cost to you.

What The Plan Pays

If you go to a VSP Panel Doctor, the plan will pay a portion of the following services and supplies, after you meet the deductible:
  • Complete vision examination every 12 months
  • One pair of lenses every 12 months (if needed)
  • One pair of frames every 12 months, including fitting and adjustments (if needed)
  • An additional pair of glasses at a 20% discount (you must use the VSP member doctor who provided the examinations)
    Contact Lenses

Contact lenses are considered medically necessary under the following conditions:
  • Following cataract surgery
  • To correct extreme visual acuity problems that cannot be corrected with glasses
  • Certain conditions of anisometropia
  • Keratoconus

When VSP Panel Doctors receive approval for such cases, the contact lenses and services are fully covered by VSP.

If you or a covered family member chooses contact lenses for cosmetic reasons, VSP will pay $110 toward the cost of these lenses.
