What You Pay
You pay the first $10 in covered expenses per person for each eye examination, and
an additional $25 if the doctor prescribes lenses, or frames, or both, in any 12
month period and if you use a Vision Service Plan provider. Oversized lenses, designer
frames, tints, etc., can be obtained at an additional cost to you.
What The Plan Pays
If you go to a VSP Panel Doctor, the plan will pay a portion of the following services
and supplies, after you meet the deductible:
Complete vision examination every 12 months
One pair of lenses every 12 months (if needed)
One pair of frames every 12 months, including fitting and adjustments (if needed)
An additional pair of glasses at a 20% discount (you must use the VSP member
doctor who provided the examinations)
Contact Lenses
Contact lenses are considered medically necessary under the following conditions:
Following cataract surgery
To correct extreme visual acuity problems that cannot be corrected with glasses
Certain conditions of anisometropia
Keratoconus
When VSP Panel Doctors receive approval for such cases, the contact lenses and
services are fully covered by VSP.
If you or a covered family member chooses contact lenses for cosmetic reasons, VSP
will pay $110 toward the cost of these lenses.
